
Very rare are the tragedies that simply are that: tragic. For almost every tragedy, there are a list of legitamate “should haves” that if followed could have prevented a catastrophe. The recent DC Metro crash is no different. The offending train was apparently recommended to be replaced years ago and was not. The offending train is not likely to have a data recorder, again, because it was not replaced as was reccomended. The most likely reason it was not replaced? Money.
Sadly, operator error is also a likely contributor, but, regardless, the system could have done more. As with most safety systems, action doesn’t occur until someone dies. Hopefully this tragedy will prevent future deaths by ensuring that METRO acts and complies or creates needed standards.
